New patient evaluation: $400

  • ~60 minutes (~50 minute interview + 10-20 min for MD documentation)

  • CPT code 90792

Follow-up (medication management & therapy): $325

  • ~60 minutes (50 minutes interview + 10-20 minutes for MD documentation)

  • CPT code 99213 or 99214 + 90836

Follow-up (medication management): $200

  • ~30 minutes (25 minute interview + 10-15 minutes for MD documentation)

  • CPT Code 99213 or 99214 + 90833

Second opinion or consultation for other MD: $475

  • ~90-120 minutes (includes time spent reviewing your records and potentially chatting with other providers. face-to-face interview portion is typically 50-60 minutes)

  • CPT code 90792

Payment is expected at the time of service. Credit cards are preferred. Cash, checks, and credit cards are accepted. Receipt/super-bills for insurance are provided.

Feel free to inquire about sliding-scale fee options, which are available under certain circumstances. 

Insurance Reimbursement

Many of my patients get reimbursed by their insurance providers even though I am out-of-network.

I bill patients directly and would provide you with a receipt called a "superbill" that you can give to your insurance company after the visit. Doing so can help you get reimbursed from your insurer.

We do not accept Medicare or Medicaid patients at this time

Laws prevent us from accepting direct payment from any Medicare or Medicaid patient.
